Clinic / Office Policies

We are committed to providing continuous and comprehensive primary care to our patients, from infants to the elderly, with an emphasis on prevention.

 

We believe that by working in a group, we can provide better coverage for our patients.  Also, because of our diverse backgrounds and interests, we can benefit from each other's experiences and improve patient care.

 

Yarmouth Medical Group is located at 21 Yarmouth Street, Guelph, Ontario, N1H 4G2.  Our office is wheelchair accessible and on street parking is available free for two hours daily with the Baker Street parking lot at the rear of the building. 

 

The office is open from Monday to Friday from 9:00am - 12noon and 1:30pm to 5:00pm, we close for lunch between 12 noon and 1:30pm each day.

 

APPOINTMENTS:

 

All visits to see your physician are by appointment only and will be booked with your regular physician.  If you appointment is deemed urgent or if your physician is not in the office, you may be booked with another physician in the practice or with the nurse practitioner.

 

Regular appointments are normally dedicated to addressing a single problem, but we are happy to deal with multiple issues at one appointment if you book adequate time for this purpose.  When making an appointment, please inform the receptionist of the nature of your problem(s) so they can book the appropriate amount of time for your visit.  Also please inform the staff when booking an appointment if you will need a form or note completed. 

 

The appointment you have means that we have reserved time for you.  If you miss an appointment without 24 hours notice or a reasonable explanation, we will bill you at the Ontario Medical Associated recommended rates, which are posted in the waiting room.

 

AFTER HOURS:

 

As a member of the FHO, you are entitled to have access to medical advice 24 hours a day, 7 days a week.  When the office is closed the telephone system will provide detailed instructions on how to reach the telehealth nurse at 1-866-553-7205 and the hours that the FHO clinic operates.

In addition, we offer after hours walk-in clinic at 35 Harvard Road( it is called Spreed River FHO at Unit 19 2 doors west of the pharmacy), 6 PM to 8 PM from Monday to Friday, 1200 noon to 2 PM Sat/Sun/ holidays. Be sure you go our  walk-in clinic, not to the  one inside ot pharmacy which is not ours.
If you feel your condition is emergence, please go to ER near you directly.

 

In return for our obligations to be available to you around the clock, your enrollment in the FHO means that you agree to always call us first and to stay away from other "walk in" clinics or "after hours" clinics since we already provide to you.  Please note that if you do go to one of these clinics OHIP payment to your clinic doctor will be deducted from your doctor's earnings and a report of the deduction will be sent to your doctor. 

 

PHONE CALLS:

 

Phone calls are handled by our office staff.  If you wish to speak to a doctor it is usually best to make an appointment so that the problem can be properly assessed and addressed.  In order to allow us to provide the best possible care, please do not ask for a diagnosis or a new prescription over the telephone.

 

HOSPITALIZATION:

 

If you are admitted to hospital, please notify our office.  We try to keep abreast of patients when admitted to hospital to assure proper continuity of care. 

 

PRESCRIPTIONS:

 

Please take your medications exactly as prescribed for best results.  If you suspect a mix up in your prescription or a reaction to a drug, please notify us immediately.  Managing your medication is both safer and easier if you keep all your prescriptions with one pharmacy.  We also ask you to bring your medications with you to each office visit to help assure that there is no confusion in your drug therapy.  Please do not forget to inform us of alternative treatments you may be receiving (e.g. herbal or homeopathic medications). 

 

The doctor will give written prescriptions for your medication for the maximum length of time, including repeats, that she feels can be allowed to safely pass without reassessing your medical status.  If you require a repeat prescription please ask you pharmacy to fax in the prescription request minimally 2 weeks earlier. 

 

LAB & X-RAY RESULTS:

 

The great volume of test results makes it impossible to call patients with every normal result.  You will, of course, be informed of significant abnormal results and  arrange for appropriate follow up.  If you wish, you may make an appointment to review your results.

 

HOUSECALLS:

 

Your physician may make house calls, but limit this service to people who are chronically housebound.  If you need emergency care, you should be transported to hospital where proper and life-saving facilities are available.

 

CONSULTATIONS WITH SPECIALIST:

 

Whenever the opinion or skills of a specialist are in the best interest of your health, the doctor will write a consultation request and the staff will arrange the appointment.   In most cases your family doctor can provide the assessment and treatment that you are seeking. Please note that some specialist appointments may take longer than others to arrange.

 

PREVENTATIVE SERVICES:

 

When our records indicate that you are overdue to receive recommended immunizations or tests that are used to detect disease in its earliest stages, we will contact you (either by phone or mail) recommending that you make an appointment for the test or vaccination. 

 
 

INSURED SERVICES:

(Services covered by OHIP)

 

You must present a valid health card each time you come to the office.  Patients who do not present a valid health card will be asked for payment.  Patients covered by Quebec health plans must pay for their appointment at the time of their visit and submit their claim for payment to their relative health plan.

 

UNINSURED SERVICES:

(Services not covered by OHIP)

 

For services not covered by provincial health plans, current charges are posted in the waiting room.

 

MEDICAL STUDENTS AND RESIDENTS IN OFFICE:

 

Occasionally there will be a medical student or resident spending several weeks accompanying the doctors in the office.  A full explanation of the teaching arrangement will be given to you should there be a student doctor or resident present.  Your participation in their learning would be optional.

 

CONFIDENTIALITY:

 

The confidentiality of your health information is of the utmost concern to us.  Our office is fully compliant with Ontario's Personal Health Information Protection Act, and our computerized records system is meticulously protected against loss of data and unauthorized access.

 

Under no circumstances will any details of your health be released.  Should you require details of your medical condition to be shared with a third party (relative, insurance company etc.) we require written authorization form the patient.  When calling your home, we will not leave any message other than to call our office unless we have been previously told that we may communicate with you through a family member or friend.

 

KEEP US UP TO DATE:

 

If you move, change work or home phone numbers, or get a new OHIP card, please be sure to notify us so that we have current contact information for you.  We can only get in touch with you regarding important health issues - such as abnormal test results, overdue immunizations or preventative tests, and new warnings about medications - if we have your current phone numbers and address.
